With some new patients, their nervousness about submitting to a complete adult physical examination, makes it difficult to assess their condition. The life circumstances and medical history collection that I conduct at the onset of the exam takes on the characteristics of a “tug of war” when a patient is nervous and clams up about their history. Martin Grande, age 25, was relaxed and easy throughout his physical examination. Martin’s last physical was performed when he began college as it was a requirement as a member of the school’s basketball team and it was performed at his family doctor’s office. Martin stands 5’10″ tall, weighs in at a trim 155 pounds and his flaccid penis at 5 1/2″ in length rested comfortably against his thigh as I posed the personal history questions to him. Martin has a steady girlfriend but his wandering eye has strayed off of the path to other women 4-5 times over the past 6 month period. Martin is playing with a loaded gun as he admits that he refrains from any condom usage during his sexual liaisons. When fully aroused, such as he was during his first prostate examination, Martin’s penis is 8 inches in length with a circumference of 6 inches. Some of my male patients have never experienced any rectal stimulation in their private lives so even the slightest bit of stimulation, like a thermometer, produces a bit of a reaction. While waiting for the thermometer to measure an accurate reading, I observed that Martin’s penis was in a semi-erect state. The introduction of my finger into his tight rectum and a bit of prostate massage was all that was required to bring Martin to a fully aroused condition. I continued Martin’s prostate exploration further in a new position; one in which is legs were lifted and knees withdrawn and resting against his chest. In this position, his legs were spread open and his anus was fully exposed and open for a much deeper exploration. I did note that Martin blood pressure is a bit higher than what it should be. He mentioned that his whole family has an issue with high blood pressure, which I did note on his medical chart. I allowed Martin to continue on his own to produce a semen sample to bring today’s adult physical to a tasty conclusion.
