Sometimes you get a mental picture of someone based on their profession. So prior to meeting, 20 year old Steele Grear, I had this picture of a typical commercial truck driver in mind. Further, his company also requires random drug testing as an employment condition. Steele, being of the youngest drivers employed by his firm, always seems to draw the attention of management for this testing.

Everything about Steele’s exam proceeded according to my plans and he was an easy, conversational patient. After his prostate examination, Steele confided in me that he an intestinal disorder a few months prior and his attending physician did perform an anoscopic exam on him as part of that examination. Something that is useful to know for a follow up visit.

Steele concluded today’s office visit by providing a very ample semen specimen.

Before landing on my exam table, 22 year old, Greg Walling has done tours of duty in both Iraq and Afghanistan. He sustained some injuries and did require some medical attention as he was hit by shrapnel. This ex-Marine began bodybuilding at age 14, and now competes competitively in strongman competitions. Questioned about his workout regimen, Greg told me that he is in the gym daily, regularly spending over 5 hours on each of his workouts, which include dead lifting over 700 pounds. He does perform his workouts with a designated spotter, someone that can assist or steady him, should he lose control of the massive amount of weight that he is lifting. His spotter is also a champion bodybuilder, in his own rite.

Based on his life experiences and his abilities, you might expected Greg to have given me a host of problems, and that would be extremely far from the truth. Greg was a very agreeable and easy patient to examine. By the way, did I also mention he is a third degree black belt ? As you might expect, I started Greg’s exam a little cautiously, I mean, let’s face it, I don’t want to be a trophy on his fireplace mantle. As the exam progressed, Greg could not have been more accommodating. I was not the first, though, to give him a prostate examination, as the Marine Corps did take that distinction from me, nor was I the first to give him a urethral swab, as it turns out, that the Marines did that as well.

The Marines did not require a semen sample at any of their exams, but I sure do and Greg fired off a good one!

At age 18, Junior Esperanza has returned for his follow up examination.Beginning in his later teen years, Junior has taken up Mixed Martial Arts (MMA) as his main athletic endeavor. His body has been stripped of all visible body hair (chest, underarms, pubic area, buttocks) as befitting a fighting machine.

Junior is well blessed in the area of his maleness. As I used the stethoscope, I noticed that my young athlete’s breathing had become a bit more labored. Glancing down lower, I also became aware that he had also sprung up a full erection. We had just completed a series of questions about his past sexual history and most likely those answers stirred up some earlier, vivid memories.

I took Reed’s temperature using a rectal thermometer. Some previous patients have reacted in much the same way as Reed. Insertion of a thermometer through a sphincter clenched in fear becomes a major ordeal. So in the interest of moving forward, I omitted a prostate exam in favor of taking a full urine specimen and a urethral culture from my patient. The urethral culture did cause him quite a bit of anguish but far less discomfort then a prostate examination would have caused.
