My new patient, Steele Grear, is a bit of an adrenaline junkie, and it was his interest in riding dirt bikes that landed him back onto my exam table so quickly after his initial exam only one week before. This 20 year old patient visits his gym daily with a dedication to his workout routine so his feelings of being invincible carried over to his dirt biking adventures. A nasty, up ended spill from his dirt bike, which caused him to land flat onto his back, also landed him, flat on his back onto my exam table.

I subjected Steele to another full battery of testing, in addition that is, to another full medical examination. On this visit, I also took a urine sample and another urethral swab, and also, to be sure there were no internal injuries, a full anoscopic exam as well.

I did not detect any serious damage to his extremely toned body however, Steele did report an area of tenderness at the base of his spine. I made time in my hectic schedule to provide my new patient some intense muscle massage therapy, applying aloe vera gel to his most initimate body parts, which culminated in another full semen specimen.

He was the only freshmen at his school to ever letter at a sport; which for him was swimming. Nineteen year old, Jackson Taylor has a naturally smooth body that as a swimmmer would be beneficial. When questioned, he told me that he has only developed a couple of stray hairs that have appeared near the center of his chest and he regular shaves them. Updating Wade’s patient chart, I found that he is keeping an extremely active sex life. It is not surprising that he has been caught in the act of masturbating several times, over the years, from various friends and family members. The most amusing story he related pertained to being caught by the school nurse. Wade, clad only in his undies, was waiting his turn for a school hosted physical examination. I rapidly moved through the steps of his exam which would ultimately lead to the removal of his boxers. When requested, Michael stood up and slipped them down and off. As I examined his scrotum, I provided information to him about what I was doing and how it was important for him to continue on his own each month to examine his own testicles. His penis began to lengthen due to my manipulations.

Kneeling on the exam table, I first palpated his spine and lower back. Reaching for both the thermometer and lubricant, I was able to secure entry to his very tight rectum after several attempts to do so. Michael clearly was not used to this type of intrusion. I fully expected some resistance but owing to his polite, country boy manner, I was able to slide my lubricated finger deep into his bowels to give him his first prostate examination. Keith has some serious dental issues as he is missing most of the upper plate of his teeth. From what little that I have gathered about him thus far, he has had a few brushes with law enforcement and that coupled with various bar room brawls, does not lend itself to good dental health.

Keith’s body matured at a normal rate during his teen years and he was slightly ahead of his peers on the secondary traits of his adolescent development. He remembers dark chest hairs appearing on his body during his 15th and 16th years. The remainder of Keith’s body is in its natural state. The pattern of chest hairs now have fully taken root and a lush growth of hairs covering his pubic area, his genitals, legs and buttocks.

Keith’s penis began to plump a bit as I started to move into the lower quadrant of his body. I could discern some movement in the encased genitals which were tightly held by briefs. Sliding them off and removing them, I had Keith’s naked form step onto the scale to record a measurement of 162 pounds on his 5’10″ frame. Standing at the exam table now, I began Keith’s genital exam. His testicles are about the size of pecans and I meticulously examined each testis and explained to him what I was checking for. His scrotum is somewhat retracted indicating he is either cold or nervous.

Mounted on the exam table, I palpated his shoulders and spine, parted open his hairy buttocks, and then lubricated a thermometer in preparation of a rectal temperature reading. I succeeded in fully inserting the thermometer and received a very loud, Oh oh oh…… from Keith. There was no indication of pleasure, just pain, as I continued and provided Keith with his first memory of a prostate examination. Today’s office visit concluded with an assisted semen extraction.

When I asked him what sport that he participated in while he was in high school, 18 year old Craig Kimble told me that it was football. He has the “soap opera” star stubble of a beard on his chin, a tanned body, bright white teeth which form an engaging smile, and a closely trimmed sculptured hair cut which frame his good looking features. Which position did you play, I asked. To which he replied, quarterback. Of course, with his looks and smile, that was the logical choice.

During his school years, Craig received his sport-related physicals  on school property, with privacy curtains assembled about the gymnasium. Craig and his teammates stood around clad in their boxers, waiting their turn to be subjected by the groping from the school district doctor. Craig was able to maintain his composure at the school hosted exam and with that memory still fresh on his mind, he was also able to stay calm during my exam as well. He did however, have an embarassing school incident when his teenaged member sprung to a full erection in the shower room after football practice.

My exams tend to be a bit more thorough then what any athlete would normally experience during a school sponsored athletic physical. I did take the time the to explain the proper method for a testicular self-examination, an activity that should be conducted on a maturing 18 year old patient, whenever possible to do so.

Craig received his first “doctor” performed rectal examination. A lot of information can be obtained just from the observation of a patient’s body language throughout an examination. Craig instinctively knew how to mount and position himself on the exam table. His buttocks parted open and revealed a shaved puckered anus, awaiting a thorough exploration. For most patients, the insertion of my finger and the probing of their most intimate parts are normally greeted with sharp intakes of breath followed by grunts and other guttural sounds of pain. My exploration into Craig was welcomed with a smile and a bigger smile when I confirmed that I was indeed massaging his prostate with my encased finger.

I noticed a clear drop of seminal fluid clinging to the tip of Craig’s penis as I began the process of collecting a urethral sample. The insertion of the cotton tip instrument into his urethra did illicit a grimace followed by a sharp grunt of pain, followed by the welcome release when I was finished and withdrew it.

For the exam conclusion, Craig continued on his own and splattered himself with a large semen sample which he was able to collect in the specimen dish for our lab.

He is slight of stature, weighing in at a mere 112 pounds and standing tall at 5″08″. In school, David took part in gymnastics and soccer, which owing to his body frame, were probably good choices for him. His gymnastic specialty was the pommel horse.

David is straight, has never had a male sexual experience, but there is something decidedly delicate about him. If talking could be considered a sport, David would most likely be an all star as he is an eager conversationalist. David’s body is naturally smooth, no chest hair development whatsoever with wisps of underarm hair. His calves exhibit hair growth while his pubic region has been neatly clipped and shaved with a mound surrounding his maleness.

David has an uncircumcised penis and while I fully meant to examine it in its repose, I was never able to do so. Upon removing his boxers to start the genital exam, David was already in a semi-rigid condition. I few harmless and casual bumps against his maleness and I was rewarded with a fully aroused penis. David’s penis curves in a large, downward arch, one in which his female companions must have found as a desirable position to give him oral gratification. I instructed David in the self-testicular exam procedures before instructing him to return to the examination table.

Positioning him onto his hands and knees, I first palpate his spine and lower back, noting how barren and devoid of hair the crevice between his buttocks, truly is. Parting his buttocks ever so slightly, the pink flesh of his anus displayed itself. David was an exceptionally docile patient and while it was first time to be examined in such a manner, I was able to fully inspect and probe his rectum with first my finger and then an anoscope. His penis was freely leaking seminal fluid which, while it still gave him discomfort, did allow for the easy insertion and collection of a urethral specimen.

David concluded today’s office visit in a solitary manner and produced a creamy white specimen for our laboratory.

Now in his second year of college, 19 year old Dustin Gaines came in to my office today for his first adult physical examination. Actually, it is his first physical exam since his body has matured. Dustin’s last physical took place when he was required to get a medical release to play sports in middle school and at that exam the doctor neglected to do a genital examination.

Dustin only played one year of basketball for his school team but now maintains his body through exercise achieved running track at his college. Dustin has accomplished his goal. His body is a trim 132 pounds on his 5’04″ frame.

Dustin was both a little nervous and apprehensive at today’s office visit.. It has been over 6 years since he has had a physical exam and his earlier exam was extremely basic and far less invasive as his exam today would prove to be. Nervous though he was, Dustin was a willing subject and I took the opportunity today discover all of his hidden secrets.

Visually inspecting his body, Dustin shaves what little hair that developed on his chest. I noted a tiny shadow of dark hair follicles around each of his nipples. His underarms, while exhibiting hair growth, have been trimmed to a very short length. This technique also is duplicated in his pubic area, genital hair has been shorn to a very short length as are the hairs covering each of the orbs comprising his buttocks.

For many, nervousness shows itself with a nervous twitch or biting on their lips. Some nervous traits also manifest themselves lower on the body and with Dustin, all traits were in play today. Upon pulling open his boxers at the waistband, a uniquely shaped, erect penis appeared. Examining Dustin’s penis, I marveled at its shape. It is 6 1/2 inches while erect, it arches like a slice of honeydew melon and its tip reminds me of a scepter that an ancient king would use to command his subjects.

Instinctively, Dustin knew the proper form to position his body for his first prostate examination. His puckered anus was clearly visible and allowed for easy insertion of first: a rectal thermometer, my probing finger and finally an anoscope. Dustin, as it turns out, had been correct to be apprehensive about today’s exam. He received a very thorough exam which culminated in the production of what appeared to be a very tasty specimen.

Before he climbed onto my exam table, 24 year old Richie Negron spent 6 years on active duty with the U.S. Army. Today, he visits my office for his first civilian physical exam since leaving the military last March. I always appreciate when I have the opportunity to examine a former serviceman as they are generally very punctual for their appointments and accustomed to their nakedness in front of another male and so it was with Richie.

In school, Richie was a member of the school’s football team (cornerback) and basketball team (point guard). All his previous sports related physical exams were done at a local clinic and while his mom took him to those exams, she did allow him the privacy and remained in the waiting room. Richie admitted to feeling a little apprehensive during one of those school exams, when instructed to undress by the female nurse, but he was able to maintain his composure and keep his penis in a flaccid state.

A naked weigh in revealed that Richie is at a fairly solid 188 pounds on his 5’10″ frame. During my initial consultation, I found out that Richie’s body stayed fairly devoid of most injuries during his active sporting years however he did suffer a slight fracture of his wrist while rollerblading which occurred during an awkward stop.

Richie keeps most of his body trimmed back using an electric trimmer however there are a few small patches of hair that remain on his pectorals and around his naval. Richie’s trimming included his pubic region and the short stubby hairs protruding through the follicles are a testament to that activity. Like his teenage exam, Richie maintained composure throughout today’s physical. His penis in a flaccid state was measured at 4 inches and then again later at 7 inches while erect.

Richie received his first prostate exam today and while unprepared for it, he was tight, yet cooperative, and admitted to experiencing the arousal factor when his prostate was purposely stimulated. Additionally, I did take a urethral culture during this exam and Richie admitted that he had been given one previously at age 15 after he informed his regular doctor that he was sexually active.

Instructed to complete the exam with a semen specimen, Richie had a difficult time of it. Achieving and then maintaining an erection in the strange surroundings clearly was an issue for him but luckily I was able to lend a hand when required to do so.

I think my general philsophy on life is that you “win some and you lose some”. Luckily for me (and ultimately you, the viewer), I tend to win more than I lose. I will put 22 year old, James Kellogg in the loss column for a few reasons.

Visually, he is spectacular, 6’03″, closely cropped blond hair, and blue eyes. For the most part, James keeps his body natural; hairy chest, abdomen, underarms, legs and buttocks. He does shave his pubic area but the view of him bent over on the exam table and the curly blond hairs growing from the crevice between his buttocks more than makes up for the barren genital area.

James was pretty much a school jock and while he was a star baseball pitcher at his high school (before suffering a season ending injury), he also was a member of the football and basketball teams as well. His sports physicals took place on the school property and they were conducted in a solitary fashion in the school gymasium or trainer’s room with only the patient and the school appointed doctor present.

My exam with James was proceeding very well until I began the posterior portion. With him positioned on his hands and knees, I was able to record a body temperature using the rectal thermometer. Continuing on to a prostate exam was when the troubles began. I was able to insert the tip of finger into his rectum and just as I was about to penetrate the outer ring, James said, no, no, no……and then knelt upright on the exam table. I subsequenly withdrew and at this point, my involvment in the exam process pretty much ceased.

James was able to continue (in a solitary fashion) after I left the exam room to produce a semen specimen. More than likely, this will be James’ sole visit to my examination office.
